Kingston Hospital has apologised after a damning three-day inquest into the death of four-year-old Freya Wells, from Wallington.

Freya’s death was the result of “flagrant and extreme” failures by staff to spot that she was in septic shock both when she was admitted and on Sunshine ward, West London Coroner’s Court heard.

Dr Rosita Ibrahim, a registrar in charge of Freya’s care, was “out of her depth”, coroner Dr Sean Cummings said.

Jane Wilson, Kingston Hospital’s medical director, said: “Our thoughts and sympathies are with Freya’s family; we are very sorry for the sad and tragic loss of Freya in November 2012.

“We carried out a full investigation following Freya’s death and made a number of changes within paediatrics to ensure that concerns about a child’s condition are always escalated appropriately.

“We can confirm Dr Ibrahim has not worked at Kingston Hospital since February 2013 when she moved on to her next training post as planned.”

In a statement outside court today, Freya’s family welcomed Dr Cummings’ proposal to ask the General Medical Council to review Dr Ibrahim’s licence.

They also paid tribute to the four-year-old.
