Mental health patients have been barred from a courtyard used to access a tall chimney where a 20-year-old climbed and fell from, it has emerged.

More than a year after Adam Withers, 20, climbed up and fell from the chimney at Epsom Hospital, the trust responsible for his care has still not released findings from their investigation into his death.

But Surrey and Borders Partnership Trust (SABP) released details on "preventative work" taken following his fall, which sparked calls for urgent action to prevent a repeat of the tragedy.

The trust said the courtyard used to access the chimney is no longer an outdoor space for people on the ward. Work has also been done to ensure drainpipes and ledges cannot be climbed.

It said new gardens have been created for each ward, adding: "These have been designed to improve the experience of people staying on the wards and prevent them from accessing the roof."

Surrey and Borders has completed its internal investigation into Mr Withers’ death in consultation with his family and shared it with "all those involved" and NHS commissioners.

When asked by this newspaper what the findings were, the trust said: "We are not in a position to share this report especially as it is yet to be put before a coroner, but we can confirm that we have completed a programme of preventative work following this tragic incident."

It added: "The learnings from our investigation have been shared across the organisation through our ongoing Lessons Learned staff training programme and we are working to reduce the number of people absconding from our wards."

Just after Mr Withers’ death on May 9, his grieving friends launched an appeal to ensure that all hospital premises are a safe place for all patients.

They said: "Adam was very ill having spent a month in hospital before he died. We, his many friends, want to prevent the possibility of any such recurrence in Adam’s memory.

"As a priority, action must be taken to ensure that patients in the care of a hospital’s mental health unit are maintained in a safe and secure environment.
